In caso di riscontrata positività a Sars-CoV2  la segnalazione di soggetto positivo deve essere inviata  all'indirizzo sicurezzacovid19@unipd.it

La comunicazione può venire inoltrata:
- dalla struttura che ne ha ricevuto la segnalazione;
- direttamente dalla persona interessata (dipendente/lavoratore o studente positivo).

Al fine di permettere un rapido tracciamento, la comunicazione deve contenere, per quanto possibile, le seguenti informazioni:  

• Referto del tampone rapido e/o molecolare (da allegare alla comunicazione stessa)
• Nome e Cognome
• Data e luogo di nascita
• Indirizzo di domicilio
• Codice fiscale
• Numero di cellulare o altro contatto telefonico
• Ruolo ricoperto presso l'Ateneo 
• Struttura di afferenza
• Se sintomatico, quando sono iniziati i sintomi
• Quando è stata l'ultima volta che è stato in Ateneo e in quali/e edifici/o
• Elenco nominativo dei possibili contatti stretti universitari (es. soggetti coi quali si è pranzato), allegando il loro indirizzo email di riferimento, quando noto 

A seguito della segnalazione l'Ateneo provvede ad attivare le conseguenti misure previste dalla norma e dal protocollo di tracciamento.

In case of a person positive to Sars-CoV2, please note that the relevant information must be sent  to sicurezzacovid19@unipd.it

The communication can be sent by:
- the structure that received the report;
- directly by the person concerned (positive employee / worker or student).

In order to allow rapid tracking, the communication must contain, as far as possible, the following information:

• Rapid and / or molecular swab report (to be attached to the communication itself)
• Name and surname
• Date and place of birth
• Address
• Fiscal Code
• Mobile number or other telephone contact
• Role held at the University
• Affiliation structure
• If symptomatic, when symptoms started
• When was the last time you were at the University and in which building
• List of possible close university contacts (e.g. subjects with whom you had lunch), attaching their reference email address, when known

 Following the report, the University activates the consequent measures provided for by the law and the tracking protocol.
